Recreation Resource Guide

To participate in any of our programs you must submit our 2008-2009 Application Form. It will remain current for 2 years. Or, if you only need to update information and you already have a full application on file, you may fill out the Application Update Form.

Please print the application, fill it out completely and send it to our STRIDE office:

After submitting the application, you will be added to our database to receive our mailings. You are responsible for reserving a spot in any of our programs. Follow instructions on our website, or call our office (518) 598-1279.
